This is a fully remote position, open to applicants in United States.
• Lead the implementation of our multi-specialty polychronic care model within a designated market.
• Work in conjunction with Monogram Health’s Multi-Specialty Platform to utilize employed specialists for providing in-home specialty care.
• Aim to enhance patient experiences, improve population health outcomes, increase provider satisfaction, and reduce costs.
• Achieve outstanding results through disease identification, evidence-based clinical pathways, and effective disease treatment.
• Manage the daily clinical and operational activities by overseeing the delivery of direct patient care, care management services, social worker assistance, and pharmacy services in the market.
• Evaluate and authorize care plans while guiding the treatment strategies for our patients.
• Partner with community physicians, healthcare facilities, and collaborators in peer-to-peer and direct patient care decision-making.
• Must be prepared and able to secure hospital privileges at necessary facilities.
• Proven experience in applying evidence-based clinical criteria.
• Background in renal care and geriatrics is essential.
• Strong management abilities and communication skills are required.
• An active, unrestricted state medical license is mandatory in every state within the market.
• Experience working with high-need Medicare Advantage and managed Medicaid populations is preferred.
• Familiarity with NCQA, HEDIS, Medicaid, Medicare, quality improvement initiatives, medical utilization management, and risk adjustment is necessary.
• Current state medical license must be free of restrictions and sanctions from Medicaid or Medicare.
• Willingness to obtain licensure in multiple states is required.
• An MD (Medical Doctor) or DO degree from an accredited medical institution is essential.
• Board Certified (BC) or Board Eligible (BE) in an ACGME approved specialty such as Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics.
