This is a fully remote position, open to applicants in Tennessee.
• The Monogram Market Physician Executive (MPE) is a mission-focused physician leader committed to enhancing the well-being, quality of life, and health outcomes for our patients.
• The MPE will spearhead our in-home multi-specialty polychronic care model within a designated market.
• Collaborating with Monogram Health’s Multi-Specialty Platform, the MPE will utilize employed specialists to provide in-home specialty care.
• In alignment with the quadruple AIM, the MPE aims to improve patient experience, population health outcomes, provider satisfaction, and reduce costs.
• The central objective for each MPE is to achieve outstanding outcomes through disease detection and evidence-based clinical pathways and treatment.
• Reporting to the Region President, the Market Physician Executive (MPE) serves as a pivotal clinical leader at Monogram Health, contributing to the formulation and oversight of clinical strategies, policies, programs, processes, protocols, guidelines, and operations aimed at enhancing patient health outcomes within the market.
• The MPE is responsible for managing daily clinical and business operations, facilitating direct patient care, care management services, social worker support, and pharmacy services within the market.
• Must be willing and able to secure hospital privileges at necessary facilities.
• Proven experience in applying evidence-based clinical criteria.
• Background in renal care and geriatrics.
• Excellent management and communication skills are essential.
• An active, unrestricted state medical license is required in each state within the market.
• Experience with high-need Medicare Advantage and managed Medicaid populations.
• Familiarity with NCQA, HEDIS, Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment.
• Current state medical license must be unrestricted and free from sanctions by Medicaid or Medicare.
• Willingness to obtain licensure in multiple states is necessary.
• MD (Medical Doctor) or DO degree from an accredited medical institution.
• Board Certified (BC) or Board Eligible (BE) in an ACGME-approved specialty such as Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics.
