This is a fully remote position, open to applicants in South Carolina.
• The Monogram Market Physician Executive (MPE) is a mission-driven physician leader committed to enhancing the well-being, quality of life, and health outcomes of our patients.
• The MPE will oversee our in-home multi-specialty polychronic care model within a designated market.
• Each market consists of 5-10 practices managed by local advanced practice providers (APP), registered nurses (RN), licensed clinical social workers (LCSW), and pharmacists (PharmD).
• Collaborate with Monogram Health’s Multi-Specialty Platform to utilize employed specialists in delivering in-home specialty care.
• The primary objective of each MPE is to achieve outstanding outcomes through disease detection, evidence-based clinical pathways, and effective disease treatment.
• Reporting to the Region President, the Market Physician Executive (MPE) serves as a vital clinical leader within Monogram Health, contributing to the development and oversight of clinical strategies, policies, programs, processes, protocols, guidelines, and operations that enhance patient health outcomes within the market.
• The MPE manages the daily clinical and business operations by providing direct patient care, care management services, social worker support, and pharmacy services within the market.
• In support of the advanced practice providers, the MPE is expected to review and approve care plans and direct treatment strategies for our patients.
• The MPE will engage with community physicians, facilities, and partners in peer-to-peer and direct patient care decision-making.
• Must be willing and able to acquire hospital privileges at the required facilities.
• Proven experience in applying evidence-based clinical criteria.
• Background in renal care and geriatrics is essential.
• Strong management and communication skills are required.
• An active, unrestricted state medical license is mandatory in each state within the market.
• Experience working with high-need Medicare Advantage and managed Medicaid populations is preferred.
• Familiarity with NCQA, HEDIS, Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment is necessary.
• Current state medical license must be free of restrictions to practice and without sanctions from Medicaid or Medicare.
• Willingness to obtain licenses in multiple states is required.
• MD (Medical Doctor) or DO degree from an accredited medical school is needed.
• Board Certified (BC) or Board Eligible (BE) in an ACGME approved specialty such as Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics is essential.
