Manager, Field Care Coordinators

This is a fully remote position, open to applicants in Florida.

📋 Description

• Act as the main point of contact for Care Coaches

• Uphold team compliance, coaching, and accountability frameworks

• Provide support to a team of 15–17 Care Coaches

• Generate and assist with metric-driven reports

• Identify obstacles and resources needed to address them

• Manage caseload ratios while reducing mileage and overtime

• Oversee patient care through assessments, data analysis, discussions with members, and active care planning

• Ensure patients are making progress towards their desired outcomes

• Make decisions regarding resources, strategies, and tactical operations for departmental projects and initiatives

• Collaborate with various departments

• Conduct briefings and area meetings

• Keep in regular contact with department managers

• Lead regions such as Jacksonville, Daytona Beach, and Deland

⛳️ Requirements

• Bachelor’s Degree in a health-related discipline

• 2+ years of experience in managed care within Medicare and/or Medicaid

• 1+ years of experience in Quality/Compliance

• Skilled in analyzing and interpreting trends

• Extensive knowledge of Microsoft Word, Excel, and Access

• Progressive experience in the health solutions sector, focusing on team leadership and management

• Proven ability to lead and inspire teams

• Must not exceed 3 hours of driving time to the regions being managed: Jacksonville, Daytona Beach, and Deland

• Preferred: proficiency in analyzing and interpreting financial trends

• Preferred: previous experience in utilization management, discharge planning, and/or home health or rehabilitation

• Preferred: background in health promotion, coaching, and wellness

• Preferred: experience with health plans

• Preferred: 1+ years of management or leadership experience

• Minimum download speed of 25 Mbps and upload speed of 10 Mbps for home internet

• Work from a dedicated area free from ongoing interruptions to safeguard member PHI/HIPAA information
