This is a fully remote position, open to applicants in Florida.
• Design, evaluate, and manage comprehensive care activities for members in long-term care.
• Create or assist in crafting tailored service care plans and provide education to members and their families/caregivers regarding available services and benefits.
• Assess member requirements and existing resources, and suggest or facilitate suitable care plans.
• Identify necessary providers, specialists, and community resources for long-term care.
• Liaise between members/families or caregivers and care provider teams to guarantee accessibility of services.
• Offer resource assistance for employment, housing, participant direction, independent living, justice, foster care, and other essential needs.
• Oversee care plans, member conditions, and outcomes while recommending updates based on evolving member needs.
• Collaborate with long-term care healthcare providers and partners to ensure that member needs are addressed.
• Collect, document, and uphold member information and care management activities for regulatory compliance.
• Conduct home and other site visits to evaluate member needs and work together with healthcare providers and partners.
• Provide or facilitate education on procedures, healthcare provider guidelines, service options, referrals, and healthcare benefits.
• Offer leadership insights on opportunities to enhance the quality of care and service delivery.
• Undertake additional duties as assigned and adhere to all policies and standards.
• Must be a resident of Broward County, Florida.
• Bachelor's degree with 2–4 years of relevant experience; or a bachelor's degree in health, human services, social work, or education with a minimum of one year of relevant experience; or a high school diploma/equivalent with three years of relevant case management experience.
• Relevant case management experience may include working with aged populations, behavioral health issues, physical or developmental disabilities, or individuals with HIV/AIDS.
• Equivalent experience may be accepted based on applicable knowledge, duties, scope, and skills that reflect the position's level.
• Bilingual in Spanish.
