LPN/LVN, RPM Experience a Plus

This is a fully remote position, open to applicants in New York.

📋 Description

• Act as the main point of contact for patients utilizing the RPM365 platform.

• Support patients with device installation and application connectivity.

• Interact with patients to promote adherence to their care plans.

• Arrange for monthly (or more frequent) check-ins and oversee patient data.

• Provide educational resources to patients.

• Alert healthcare providers regarding critical notifications.

• Participate in virtual team meetings for updates.

⛳️ Requirements

• Current nursing license in New York State

• Understanding of chronic illnesses and associated monitoring devices

• Competence with technology, computer systems, and smartphones

• Excellent verbal and written communication abilities

• Comfortable engaging with and reaching out to patients

• Highly organized, detail-oriented, and collaborative

• Self-motivated and capable of working independently
