📋 Description

• Acquire knowledge of Solace's proprietary systems, tools, technologies, and expectations, while contributing your unique insights in each interaction.

• Foster strong, trusting relationships with patients, where active listening and empathy serve as the cornerstone of every engagement.

• Identify and prioritize patients’ needs to create a comprehensive care plan that ensures consistent follow-through over time.

• Design inclusive patient care plans that holistically address social determinants of health, such as access to food resources, transportation, and support at home.

⛳️ Requirements

• Active LPN/LVN license in good standing.

• Experience working with a variety of patient populations.

• Empathy: As a seasoned LPN/LVN, you possess a deep concern for individuals and a commitment to advocate for those who are unable to speak for themselves.

• Clinical and Organizational Skills: You can manage multiple responsibilities while maintaining composure under pressure.

• Technical Adaptability: You have the ability to quickly and efficiently learn new systems.

• A Proactive Approach: When you recognize a task that requires attention, you take the initiative to address it.

• Applicants must reside in the United States.
