LPN Personal Care Coordinator I

This is a fully remote position, open to applicants in California.

📋 Description

• Conduct a monthly care management dialogue through outbound calls to designated patients to meet their healthcare needs.

• Share information about resources and services accessible to patients throughout their care journey.

• Recognize patient-specific challenges, objectives, and interventions tailored to fulfill the individual needs of each patient through actionable and timely goals.

• Ensure compliance with documentation standards, including but not limited to medical history, medications, immunizations, allergies, surgical history, and family medical history.

• Show sensitivity to a patient’s situation that may require adjustments to care plans, goals, and preferences while being mindful of obstacles to care and services.

• Offer relevant health education.

• Raise patient concerns to the triage nursing team.

• Monitor performance metrics, including proper utilization, achieving scheduled call percentages, quality scores, and maintaining scheduled hours with proper attendance.

• Valid active CA license and an active multistate license.

• Strong adherence to established clinical protocols and procedures.

• Ability to adapt and switch tasks efficiently.

• Willingness to support colleagues and contribute to the success of the team.
