This is a fully remote position, open to applicants in Ohio.
• Become a part of Aledade’s expanding clinical call center team in this fully remote, telephonic position tailored for LPNs seeking to move away from the pressures of bedside care into impactful patient advocacy.
• As an LPN - Advanced Care Planning Specialist (internally referred to as Facilitator), you will assist patients in navigating significant end-of-life care discussions and ensure their medical preferences are documented from your home office.
• This full-time role offers structured 8-hour shifts scheduled from Monday to Friday, during the hours of 8:30 a.m. to 10:00 p.m. ET, with specific shift times rotating approximately every six months to maintain team equilibrium.
• Additionally, in order to support our patients during peak times, voluntary overtime may be requested during high-volume months, providing you the chance to enhance your earnings while assisting families in their time of need.
• We embrace flexibility regarding geographic location, and the ideal candidate will be at ease working remotely across various US time zones.
• Licensed Practical Nurse (“LPN”): Certificate or Diploma in Practical Nursing
• Current and unrestricted LPN license
• A minimum of 2 years’ experience as an LPN or relevant experience
• Proven knowledge of serious medical conditions (e.g., COPD, CHF, ESRD, etc.) and their progression
• Demonstrated capability to adapt to changing priorities and fast-paced environments
• Familiarity with Google Suite (Google Meets, Google Sheets, & Google Docs); Ability to quickly learn new call center/telephony and patient documentation technologies.
• Proficiency in navigating between multiple systems while engaging with patients.
• Strong communication skills (both verbal and written) with a high level of active listening and empathy to handle sensitive conversations.
• Excellent time management skills and meticulous attention to detail
• Understanding of state-specific legal regulations regarding advance directives and Medical Power of Attorney; Familiarity with ACO and Medicare/CMS guidelines.
