This is a fully remote position, open to applicants in Washington.

📋 Description

• Evaluates, plans, and implements tailored care strategies aimed at providing members with the most suitable and least restrictive care options.

• Works in collaboration with members, families, and healthcare professionals to create a personalized care plan.

• Facilitates individual counseling and group therapy sessions for both adolescents and adults.

• Recognizes and initiates referrals to social service programs, including financial, psycho-social, community, and state support services.

• Advocates for members and their families as necessary to ensure that the needs and preferences of the patient are thoroughly represented and supported by the healthcare team.

• Applies approved clinical criteria to evaluate and determine the appropriate level of care for members.

• Records all member assessments, care plans, and referrals made.

• Accountable for meeting established goals and Key Performance Indicators (KPIs).

• Engages with the StrongWell model, taking initiative and responsibility for outcome-based care.

• Takes part in interdisciplinary team meetings and utilization management rounds, providing valuable information to support safe transitions of care.

• Encourages responsible and ethical management of company resources.

• Maintains excellent punctuality and attendance throughout work hours.


⛳️ Requirements

• LCSW, LMFT, LMHC, LPC certification in Washington.

• Experience with substance abuse populations is preferred.

• Background in maternal and substance abuse is advantageous.

• Experience working with both adults and adolescents.

• Familiarity with Medicare and Medicaid processes.

• Experience with remote work is required.
