Licensed Clinical Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Conduct psychosocial assessments and create personalized care and support plans.

• Provide counseling and crisis intervention services for both patients and their family members.

• Collaborate with interdisciplinary teams to ensure seamless continuity of care.

• Assist clients in connecting with community resources, benefits, and follow-up services.

• Ensure timely and accurate clinical documentation while adhering to regulatory compliance.

⛳️ Requirements

• Current LCSW license in good standing issued by the State Board of California.

• Master of Social Work degree from a recognized and accredited program.

• 1 to 2 years of experience in providing clinical counseling and crisis intervention in healthcare environments.

• In-depth knowledge of community resources, care coordination, and discharge planning.

• Proven ability to collaborate effectively with interdisciplinary teams.

• Experience in maintaining timely and accurate clinical documentation.

• Preferred: Familiarity with Devero.

• Preferred: Experience in home health settings.

• Preferred: Background in supporting older adults and patients post-discharge.
