
In-Home Stars Program Delivery Lead

This is a fully remote position, open to applicants in California, +3 more states.
• Identify, design, and execute programs that influence providers, members, or market leadership towards value-based relationships and enhanced quality metrics.
• Develop Stars initiatives aimed at boosting performance and increasing the clinical value of the in-home program.
• Collaborate with leaders on the implementation, planning, and enhancement of programs.
• Analyze complex data sets, trends, and insights to review and communicate program outcomes.
• Create functional strategies to enhance Stars performance.
• Guide and oversee external vendors as well as internal partners.
• Provide strategic leadership and foster collaboration to support Stars and Pharmacy operations.
• Work together with Humana leaders, external partners, and the in-home team to develop short- and long-term solutions.
• Formulate and refine frameworks for deliverables, processes, reporting, and performance tracking.
• Enhance Stars-related program components, such as referrals, screenings, member follow-up, HRAs, PCP summaries, and visit data.
• Drive tangible improvements in Stars metrics and outcomes.
• Bachelor’s degree.
• 7 or more years of experience in program management, vendor management, and/or strategy.
• Experience with Stars programs.
• Proven experience presenting to leadership.
• Strong skills in relationship building.
• Excellent analytical abilities.
• Proficient in Microsoft Office XP products, including Word, Excel, and PowerPoint.
• Demonstrated ability to effectively articulate ideas in both written and verbal formats.
• Master’s Degree in Business Administration or a related field (preferred).
• Desired experience with HEDIS.
• PMP certification is advantageous.
