Hospice Triage RN, PT

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct telephone triage for hospice patients and their families

• Evaluate patient conditions and determine suitable next steps

• Collaborate with on-call teams to ensure care and resource coordination

• Precisely document all communications and interventions

• Maintain a composed and professional demeanor when managing urgent calls.

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license

• Experience in hospice, palliative, or end-of-life care is highly preferred

• Must be proficient with technology and electronic medical records (EMR) used for documenting calls

• Capability to type documentation and notes efficiently in a fast-paced environment

• Ability to manage stress and multitask while receiving calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends)

• Excellent communication and critical thinking abilities

• Capacity to work independently in a remote setting

• This position is remote and requires consistent attendance, proactive communication, and dependable internet connectivity throughout all scheduled shifts to facilitate timely patient care coordination.
