Hospice Triage RN, PT

This is a fully remote position, open to applicants in Texas.

📋 Description

• Provide telephone triage for patients and families in hospice care.

• Evaluate patient conditions and decide on the appropriate next steps.

• Collaborate with on-call teams to ensure effective care coordination and resource allocation.

• Accurately record all communications and interventions.

• Maintain a calm and professional demeanor while managing urgent calls.

⛳️ Requirements

• Active multistate Registered Nurse (RN) license is required.

• Experience in hospice, palliative, or end-of-life care is strongly preferred.

• Must be comfortable with technology and proficient in using electronic medical records (EMR) for documentation of calls.

• Ability to type documentation and notes efficiently in a fast-paced environment.

• Must be capable of managing stress and multitasking while handling calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends).

• Strong communication and critical thinking abilities are essential.

• Ability to work independently in a remote setting.

• This position is remote and requires consistent attendance, active communication, and reliable internet connectivity during all scheduled shifts to ensure timely coordination of patient care.
