Hospice Triage RN, PT

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions to determine suitable next steps.

• Work in collaboration with on-call teams to coordinate care and resources.

• Ensure accurate documentation of all communications and interventions.

• Maintain a composed and professional demeanor while managing urgent calls.

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR) for call documentation.

• Ability to type documentation and notes efficiently in a fast-paced environment.

• Capable of managing stress and multitasking while receiving calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends).

• Excellent communication and critical thinking abilities.

• Self-sufficient in a remote work setting.

• This is a remote role that requires consistent attendance, proactive communication, and reliable internet connectivity during all scheduled shifts to facilitate timely patient care coordination.
