Hospice Triage RN, PT

This is a fully remote position, open to applicants in Arizona.

📋 Description

• Conduct telephone triage for patients and families in hospice care.

• Evaluate patient conditions and decide on the appropriate next steps.

• Work together with on-call teams to facilitate care and resource allocation.

• Ensure accurate documentation of all communications and interventions.

• Maintain a composed and professional attitude while managing urgent calls.

⛳️ Requirements

• A valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR) for documentation purposes.

• Capability and comfort in typing notes and documentation in a fast-paced setting.

• Ability to manage stress and multitask while receiving calls (minimum of 5 calls per hour on weekdays, and up to 8 per hour on weekends).

• Exceptional communication and critical thinking abilities.

• Capacity to work independently in a remote setting.

• This remote position necessitates consistent attendance, active communication, and reliable internet connectivity during all scheduled shifts to ensure timely coordination of patient care.
