Hospice and Palliative RN

This is a fully remote position, open to applicants in Arizona.

📋 Description

• Assist patients with chronic illnesses through advanced illness management, palliative care oversight, and hospice coordination.

• Assess urgent palliative care requirements and escalate them as necessary.

• Offer clinical support to regional palliative care physicians, including assistance with documentation and workflow.

• Facilitate referrals to hospice services and coordinate informational visits.

• Aid in scheduling and provide pre-visit education for patients with complex needs.

• Acquire Advance Care Planning (ACP) documentation from external offices and hospitals.

⛳️ Requirements

• Graduated from an accredited nursing program.

• Currently holds a Registered Nurse license.

• At least 2 years of experience in palliative care, geriatric home-based care, or hospice care.

• Willingness to travel to assigned market(s) up to 20% of the time.

• Self-motivated individual capable of working independently and collaboratively in a remote setting with minimal supervision.

• Proficient in Microsoft Office and web-based applications.

• Strong verbal and written communication skills.

• Previous experience in value-based care models is preferred.
