📋 Description

• Perform comprehensive patient evaluations, encompassing medical history, physical assessments, and diagnostic evaluations.

• Create individualized treatment strategies tailored to the specific needs and medical conditions of patients.

• Prescribe nutraceuticals and/or medications while monitoring their efficacy.

• Collaborate with other healthcare professionals to ensure coordinated patient care.

• Offer counseling and education to patients regarding disease prevention and healthy lifestyle choices.

• Maintain precise and current patient records utilizing electronic health record systems.

• Remain informed about medical advancements, research, and clinical trials in functional, integrative, and lifestyle medicine.

⛳️ Requirements

• NP/APRN with a specialization in Functional Medicine, Hormone Health, Anti-Aging, or Lifestyle Medicine.

• Formal training and experience in practicing Bioidentical Hormone Replacement Therapy (BHRT) is highly preferred.

• Must possess (or be able to acquire) an IL APRN and IL DEA licenses.

• Ideally seeking full-time candidates, but must be available for at least 16-20 hours per week, including 1-2 evening shifts.

• Excellent oral and written communication skills.

• Energetic and personable demeanor with a passion for assisting others.

• Commitment to a patient-first approach and ability to consistently deliver a 5-star service level.

• Familiarity with HIPAA regulations and capability to uphold patient confidentiality.
