Home Health Telehealth Registered Nurse – Part-Time

This is a fully remote position, open to applicants in United States.

📋 Description

• Conducts comprehensive triage assessments, case management, and coordination through telephonic and telehealth services for patients.

• Ensures optimal patient care coordination throughout the continuum for all assigned disease management and triage patients.

• Offers telephonic triage for all incoming calls from patients and caregivers as assigned.

• Collaborates with Clinical Supervisors, Customer Service personnel, field staff, and on-call team members to effectively coordinate care that meets patients’ needs.

• Serves as a patient advocate, facilitating care coordination both within the organization and externally.

• Engages with all healthcare providers involved in the care of Disease Management/Triage patients to ensure seamless care coordination.

⛳️ Requirements

• Must possess a current state license as a Licensed RN.

• Strong telephone and communication skills are essential.

• 2-3 years of clinical experience in the relevant field is required.

• 1-3 years of experience in home health or hospice settings is preferred.

• Familiarity with disease management and chronic disease processes is necessary.

• Previous management experience is advantageous.

• Capability to work both independently and as part of a multidisciplinary team.

• Must maintain a professional license, certificate, or registration in good standing, as applicable.
