This is a fully remote position, open to applicants in United States.

📋 Description

• Provide functional and analytical expertise for the implementation, optimization, and ongoing support of Epic Healthy Planet.

• Convert value-based care requirements into Epic Healthy Planet workflows, reporting, and analytical solutions.

• Support initiatives in population health that emphasize quality, cost, utilization, outcomes, and patient engagement.

• Develop and refine analytics for value-based contracts, accountable care, and population health programs.

• Define key performance indicators (KPIs) and actionable population health metrics in collaboration with clinical and operational stakeholders.

• Assist with reporting and analytics for CMS, HEDIS, MIPS, ACO, Medicare Advantage, Medicaid, and other value-based care programs.

• Utilize Epic Healthy Planet data to identify care gaps, high-risk populations, utilization trends, and opportunities for clinical intervention.

• Aid in the risk stratification and segmentation of patient populations.

• Create and analyze care gap, registry, quality measure, and population health reports.

• Partner with clinical teams, care managers, quality teams, and physician leadership to translate analytics into actionable interventions.

• Analyze performance relative to value-based contract benchmarks and pinpoint quality and financial improvement opportunities.

• Support the creation of population health and value-based care dashboards, along with executive reporting.

• Assist with data validation, reconciliation, testing, and troubleshooting tasks.

• Collaborate with Epic application teams, reporting and analytics teams, data analysts, and organizational leadership.

• Engage in requirements gathering, workflow analysis, solution design, testing, training, and go-live support activities.

• Evaluate population health processes and propose enhancements in workflow, analytics, and operations.

• Support data-driven initiatives aimed at improving patient outcomes while reducing avoidable utilization and costs.


⛳️ Requirements

• Minimum of 5 years of experience in healthcare IT, clinical informatics, population health, or healthcare analytics.

• At least 5 years of proven experience with Epic Healthy Planet projects.

• Experience in supporting Epic Healthy Planet implementations or major optimization efforts.

• Strong understanding of value-based care and population health analytics.

• Experience in converting clinical and operational requirements into analytical and reporting solutions.

• Knowledge of healthcare quality programs and performance measures.

• Strong analytical, problem-solving, and data interpretation capabilities.

• Ability to convey complex healthcare analytics concepts to both technical and non-technical stakeholders.

• Experience collaborating with physicians, clinical leadership, quality teams, care management, finance, and IT.

• Strong project management and documentation abilities.

• Epic certification in Healthy Planet is required.

• Preferred: Experience with Clarity and/or Caboodle.

• Preferred: SQL and healthcare data analytics skills.

• Preferred: Familiarity with Tableau, Power BI, or other enterprise analytics platforms.

• Preferred: Experience with CMS, HEDIS, ACO, MIPS, Medicare Advantage, or Medicaid value-based programs.

• Preferred: Knowledge of risk adjustment, RAF/HCC methodologies, or actuarial analytics.

• Preferred: Experience with payer-provider value-based contracts.

• Preferred: Development experience with executive dashboards and performance scorecards.

• Preferred: Experience within large health systems or integrated delivery networks.
