This is a fully remote position, open to applicants in United States.
• Conduct educational phone calls to inform members about the benefits, services, and programs available to them.
• Perform health needs assessments and guide members towards suitable population health management programs.
• Engage with members who have identified gaps in care and promote adherence to recommended actions.
• Aid members in finding providers and arranging appointments.
• Oversee system work queues effectively.
• Evaluate members for eligibility, previous case activities, recent claims, customer service inquiries, and authorization history.
• Allocate members to the clinical team for outreach and intervention calls.
• Investigate and analyze inquiries or complaints concerning processes, designations, lost incentives, and other program-related issues.
• Be prepared to work overtime as necessary.
• An associate degree in education, communication, or a health-related field, or equivalent professional experience.
• A minimum of 2 years in a customer service support role.
• Ideally, at least one year of experience in a medical office or with senior populations.
• Familiarity with medical terminology.
• Capability to work independently with little supervision or as part of a team.
• Strong interpersonal and organizational abilities.
• Effective decision-making and problem-solving skills.
• Self-driven, capable of managing multiple tasks and prioritizing effectively.
• Proficient time management skills.
• Excellent verbal and written communication skills.
• Completion of a digital literacy assessment is required.
• Various immunizations and/or related medical tests may be necessary.
