This is a fully remote position, open to applicants in United States.
• Act as the primary point of contact for patients and their caregivers.
• Foster rapport and build trusting relationships with patients and their families.
• Make outbound calls to inform patients about Sylvan Health programs, assist with enrollment, and schedule initial and follow-up telehealth visits with Registered Dietitians.
• Assist patients with inquiries regarding scheduling, telehealth consultations, and general insurance matters.
• Send follow-up communications via phone calls, SMS, email, and patient portal messaging.
• Provide prompt support for incoming patient communications, including appointment changes and visit transfers.
• Ensure outstanding customer service and demonstrate empathy in every patient interaction.
• Work collaboratively with Registered Dietitians and other members of the care team.
• Accurately document patient interactions within EMR and CRM systems.
• Resolve inquiries from patients and caregivers in a professional and efficient manner.
• Navigate multiple systems and workflows simultaneously with a keen attention to detail.
• Legally authorized to work in the U.S.
• A minimum of 2 years of experience in a patient-facing or healthcare communication role, such as Care Coordination, Patient Scheduling, or Healthcare Customer Support.
• Excellent interpersonal and communication skills, with the ability to connect with individuals from diverse backgrounds and varying levels of health literacy.
• Proven ability to manage sensitive, complex, or fast-paced situations in real time.
• Working knowledge of the U.S. healthcare system, including:
• Types of provider visits and care pathways.
• Basic insurance concepts such as deductibles, coverage, and in-network versus out-of-network care.
• General patient navigation workflows.
• Ability to communicate clearly and confidently through phone, SMS, and email.
• Strong organizational skills and meticulous attention to detail.
• Comfortable with adapting to changing workflows and operational processes.
• Capability to work efficiently across multiple software systems simultaneously.
• A Bachelor's degree is preferred, though not required.
• Previous experience or knowledge in nutrition or the Medical Nutrition Therapy field is beneficial but not mandatory.
• 1099 structure.
