Health Coach, NBHWC Certified

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform outbound telephonic outreach to patients and provide individualized coaching and support for those managing chronic conditions, including but not limited to type 2 diabetes, hypertension, and cardiovascular disease.

• Assist patients in implementing tailored care plans and reaching their objectives by delivering education and coaching centered on behavior modification, nutrition, physical activity, and self-management techniques utilizing evidence-based methods such as motivational interviewing and SMART goals.

• Carry out thorough assessments of patients’ health status, lifestyle choices, nutritional practices, and willingness to make changes.

• Educate patients on managing their conditions, adhering to medications, recognizing symptoms, and employing prevention strategies, while performing care delivery duties in alignment with Cadence clinical policies and established protocols under suitable clinical supervision.

• Track patients’ progress, adherence to treatment plans, and health outcomes through regular follow-ups and remote monitoring tools.

• Record patient interactions, outcomes, and follow-up strategies in the Cadence platform; identify obstacles to care (including social determinants) and link patients with necessary resources or internal support systems.

• Collaborate in real-time with clinicians and patient support teams using tools such as the Cadence Platform, Google Workspace, and Slack, and provide feedback to enhance workflows in a dynamic, change-driven environment.

⛳️ Requirements

• An active NBHWC health coaching certification is mandatory.

• A minimum of 1 year of experience as a health coach is essential.

• Experience in providing education and support to patients managing chronic conditions is required.

• Preparedness for remote work (including reliable high-speed WiFi) is necessary.

• Fluency in Spanish is highly preferred.

• Proficiency with AI-assisted clinical tools (such as Gemini, Open Evidence, or similar platforms) and the capability to discuss credibly with providers and patients regarding the role of AI in care delivery versus the necessity of human judgment.
