Health Coach

This is a fully remote position, open to applicants in Ohio, +3 more states.

📋 Description

• Provide education to members on Managing Medicines, Nutrition, Safety, Benefits/Resources, Mindfulness, and collaboration with the Care Team.

• Facilitate group coaching sessions for members managing chronic conditions.

• Offer information, referrals, and assistance in accessing resources, health coaching, and supportive care specifically for Medicare members.

• Encourage members to engage actively with their healthcare providers and Aetna Care Management teams.

• Record all member interactions in the designated member records.

• Exhibit empathy towards members and their caregivers.

• Resolve issues with unavailable resources and obtain support to bridge gaps for members.

• Adhere to program workflows and evaluate members’ personal action plans.

• Assess the need for additional referrals to Care Management, Behavioral Health, or other services.

⛳️ Requirements

• Minimum of 2 years experience as a Health Coach.

• An Associate’s Degree is mandatory.

• Capability to establish a direct/hardwired internet connection to a modem/router within 7 feet of the computer.

• Required internet speed of at least 25 Mbps for download and 3 Mbps for upload.

• WiFi and satellite internet connections are not accepted.

• A quiet, secure, and private home workspace that adheres to CVS Health and HIPAA guidelines.

• Must reside and work within the state and city of current residence.

• Proficient in setting up and using multiple monitors and navigating various applications.

• Effective communication through email, calendar invites, Teams messaging, and virtual meetings.

• Familiarity with Microsoft Office 365 applications or comparable Google Workspace applications.

• Ability to access secure systems such as VPN and EHR portals.

• Competence in managing strong passwords and identifying suspicious emails or links.

• Skill in independently troubleshooting common technical issues.

• Willingness to learn new skills as workplace dynamics evolve.
