This is a fully remote position, open to applicants in Texas.
• Work schedule from Monday to Friday, with no on-call duties or weekend obligations.
• Perform in-home assessments for health plan members for 3.5 days each week, including some outpatient clinical responsibilities, with 1.5 days dedicated to remote work.
• In-Home Assessments will involve collecting a comprehensive medical history, conducting physical examinations, reviewing medications, and performing necessary cognitive, depression, and safety screenings.
• Must be comfortable discussing End-of-life care.
• Conduct laboratory specimen collection in the home setting when appropriate.
• Identify diagnoses that will support active medical management and care management, with a focus on chronic disease management.
• Communicate assessment findings to the Primary Care Physician (PCP) to highlight any potential gaps in patient care.
• Provide education to patients, families, and caregivers, emphasizing close monitoring and follow-up on patient needs.
• Strong emphasis on knowledge of suitable community resources for referral.
• Adhere to all HIPAA regulations and ensure the security of protected health information (PHI).
• Minimum of one (1) year of prior clinical experience.
• Active state license as a Nurse Practitioner or Physician Assistant.
• Current Nurse Practitioner or Physician Assistant Board Certification.
• Must be a Medicare Part D Prescriber.
• Possess a valid driver’s license and current auto insurance.
• Furnishing number required upon hire.
• NPI Number required upon start.
• DEA Number to be obtained within 6 months of starting.
• Board Certification from AANP or ANCC required within 6 months of starting.
• Valid Basic Life Support (BLS) certification required upon start.
• Valid Advanced Cardiovascular Life Support (ACLS) certification preferred.
• Familiarity with current clinical standards of care.
• Ability to work independently.
• Experience with geriatric patients is preferred.
• Previous home care experience is preferred.
• Strong administrative, organizational, and verbal communication skills are preferred.
• Prior experience with EPIC is preferred but not necessary.
