This is a fully remote position, open to applicants in Florida, +1 more state.
• Identify available resources and community support for families or caregivers.
• Offer information, referrals, and non-clinical assistance to connect families with medical, psychosocial, and basic-needs services.
• Empower families to advocate for themselves through education, guidance, and supportive tools.
• Assist in accessing programs and supports that address psychosocial needs within Imagine Pediatrics and the wider community.
• Advocate with community agencies on behalf of clients and families to minimize service gaps and barriers.
• Collaborate with internal staff and providers to facilitate system navigation, including eligibility, pediatrics, behavioral health, and public health.
• Help create an internal community resource library, excluding clinical library materials.
• Connect families to community-based and school-based services.
• Network with community resources to enhance knowledge of available services.
• Document memoranda of understanding for partnerships with outside community agencies.
• Build effective working relationships with interdisciplinary care teams.
• Participate in scheduled interdisciplinary consultations regarding administrative and non-clinical resources.
• Perform additional duties as assigned.
• Bachelor’s degree or higher in a healthcare, behavioral health, or social services-related field is preferred.
• Certification as a Community Health Worker is highly desirable.
• Familiarity with navigating social service agencies.
• Reliable transportation is required.
• Valid Driver's License is necessary.
• Ability to provide a copy of current auto insurance certificate.
• Experience with resourcing and telephonic services is essential.
• Bilingual in Spanish is preferred.
• Availability to work Monday to Friday from 8 am to 5 pm.
