Family Practice Nurse Practitioner – Massachusetts Licensed

This is a fully remote position, open to applicants in Louisiana, +1 more state.

📋 Description

• Assess the daily caseload and prepare technology for patient services.

• Utilize technology to navigate the daily schedule and collaborate with clinical in-home team members visiting patients’ residences.

• Create welcoming connections with a diverse patient population and gather pertinent information.

• Conduct virtual wellness consultations and health assessments for adults and/or children.

• Execute health risk assessments, review medications, and perform cognitive screenings.

• Offer patients educational resources regarding their health and well-being.

• Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and cater to patient needs.

• Deliver care coordination and management, encompassing preventive care interventions, medication management, specialist referrals, community resources, and documentation.

• Employ autonomous clinical judgment while upholding high-quality standards.

⛳️ Requirements

• Must be Board Certified as a Family Nurse Practitioner.

• Hold an active Family Nurse Practitioner license in Louisiana.

• Previous experience in conducting wellness visits is essential.

• Demonstrate professionalism, integrity, accountability, confidentiality, care, and compassion.

• Must possess a willingness to engage in last-minute problem-solving and innovative thinking.

• Be technologically proficient and comfortable using laptops or mobile devices for documentation.

• Familiarity with HIPAA-secure messaging applications for care coordination is required.

• Strong written and verbal communication skills are necessary.

• Ability to work autonomously or collaboratively in a team setting.

• Successfully pass a national background check and valid clinical license verification.

• Must currently have prescriptive authority in the states where licensed to practice.

• Must be a Medicaid-enrolled provider.

• Virtual experience as a Family Nurse Practitioner is requested in the application.

• Prior experience with Medicare and/or Medicaid Annual Wellness Visits is requested in the application.
