Family Nurse Practitioner

This is a fully remote position, open to applicants in New Mexico, +1 more state.

📋 Description

• Assess the daily caseload and ready technology and tools for patient service.

• Utilize modern technology to manage the daily schedule efficiently.

• Collaborate with clinical in-home team members (Sprinters) to visit patients at their residences.

• Establish friendly connections with a diverse patient population.

• Gather pertinent patient information and conduct virtual wellness visits and health assessments for adults and/or pediatrics.

• Carry out health risk assessments, medication evaluations, and cognitive screenings.

• Educate patients regarding their health and well-being.

• Collaborate with Sprinters to assess vital signs, organize blood draws, and address specific patient needs.

• Coordinate and manage care, which includes preventive care interventions, medication management, referrals, community resources, and documentation of findings.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Active Family Nurse Practitioner License in Michigan.

• Previous experience in conducting wellness visits.

• Demonstrates professionalism, integrity, accountability, confidentiality, care, and compassion.

• Eagerness to creatively solve problems in an innovative environment.

• Proficient with technology, including laptops, mobile devices, charting tools, and HIPAA-compliant messaging applications.

• Excellent written and verbal communication abilities.

• Capable of working independently as well as collaboratively within a team.

• Must be able to pass a national background check and valid clinical license verification.
