This is a fully remote position, open to applicants in North Carolina.
• Oversee and manage services for clients engaged in the AFL program
• Act as a bridge between clients, host families, service providers, community resources, and support systems
• Aid in assessments, placement matching, and service coordination
• Create, implement, and oversee behavioral intervention and treatment plans
• Offer support, coaching, and crisis intervention to both clients and host families
• Engage in clinical team meetings and collaborate on treatment methodologies
• Ensure documentation complies with state, federal, and agency regulations
• Complete placement documentation, authorizations, attendance logs, and billing records
• Employ person-centered planning techniques to achieve client goals and outcomes
• Assist with transition planning and discharge coordination when needed
• Support outreach and recruitment initiatives, along with maintaining relationships with referral sources
• Advocate for Lutheran Services Carolinas within the community and human services network
• A Bachelor's degree in Social Work, Psychology, Human Services, or a related discipline with two years of pertinent experience, including two years in client welfare services; OR a Bachelor's degree in an unrelated field with five years of relevant experience, including two years in client welfare services; OR a Master's degree in a Human Services-related area with one year of client welfare experience
• Familiarity with child welfare and behavioral health systems
• Proficient in conducting assessments and formulating behavioral treatment plans
• Strong skills in crisis intervention and behavior management
• Knowledge of family systems, attachment theory, psychopathology, and psychotropic medications
• Understanding of person-centered planning methods
• Exceptional communication, organizational, and documentation abilities
• Capacity to work independently while engaging with a multidisciplinary team
• Strong time-management and problem-solving skills
• Must be at least 21 years old
• A valid driver's license is required
• Reliable transportation is necessary
• Willingness to travel within the assigned region for home visits and client support
• Flexibility to work occasional evenings, weekends, and on-call shifts as required
• Capability to lift and carry up to 25 pounds
• An additional $2.00 per hour for teammates who choose to opt out of benefits
