This is a fully remote position, open to applicants in Washington.
• Facilitate the completion of CHE in nephrology offices, dialysis clinics, or through telehealth; collaborate with Nurse Practitioners and clinics to prepare for in-person appointments.
• Conduct outbound calls to patients, caregivers, and clinics for scheduling CHEs, sending reminders, and rescheduling; arrange telehealth CHEs for patients unable to attend in person.
• Welcome new patients to the program and gather any new concerns from existing patients; escalate issues to the RN or Nurse Practitioner as necessary.
• Maintain precise documentation and communicate updates about patients to the care team.
• Exhibit outstanding interpersonal skills to foster trust, encourage patient participation, and navigate barriers to engagement.
• Organize outreach initiatives by urgency and significance to enhance patient impact.
• Coordinate patient care by ensuring access to eligible resources/programs and assisting with referrals and provider appointments.
• Make outbound calls and handle inbound calls from patients, providers, and Village resources.
• Complete non-clinical screenings and pathways under the supervision of the Manager; escalate to the Integrated Care Nurse when necessary.
• Assist the clinical team in guiding patients through the continuum of care by executing tasks as directed.
• Identify eligible resources for members and collaborate with Integrated Care Nurses, dialysis clinics, health plans, and other stakeholders to facilitate member enrollment in programs.
• Communicate directly with patients in a timely and professional manner.
• Maintain accurate, complete, and confidential documentation.
• Demonstrate excellent two-way communication skills, both verbal and written.
• High School Diploma or GED is required; an Associate’s or Bachelor’s Degree is preferred.
• Previous experience with IKC is required.
• Over 2 years of professional experience in customer service, patient-facing, or telephonic outreach roles is preferred.
• Prior experience in healthcare is preferred.
• Intermediate proficiency in Microsoft Word, Excel, PowerPoint, and Outlook.
• Strong verbal and written communication skills, active listening abilities, and motivational engagement techniques.
• Highly organized with excellent time management and follow-through skills.
• Ability to work both independently and collaboratively with care team members.
