This is a fully remote position, open to applicants in California.
• Achieve performance metrics, goals, and deadlines for the team while inspiring them to meet and surpass objectives.
• Oversee the daily operations within their assigned territory, which includes conducting individual performance and caseload assessments as well as monitoring productivity, growth, member engagement, quality of care, and graduation rates.
• Collaborate with community-based organizations, health plans, and various partners.
• Deliver continuous virtual and on-site coaching and development for the field team.
• Offer resources related to community-based opportunities in conjunction with the Community Impact Partnership Program (CIPP) Director.
• Guide Community Health Navigators in establishing Specific, Measurable, Achievable, Relevant, Time-based (SMART) goals with feasible interventions and in overcoming barriers and objections faced by members.
• Tackle issues raised by Community Health Navigators in a proactive and innovative manner.
• Assist in program expansion, staffing requirements, and operational improvements.
• Recruit, manage, and mentor staff.
• Oversee Profit and Loss statements for the territory.
• Ensure the prompt review and approval of payroll, expenses, and other compliance-related tasks within the territory.
• A minimum of 3-4 years of experience in managing personnel in health, social, or community services.
• High School Diploma or equivalent qualification.
• Strong analytical capabilities along with a thorough understanding and comfort with data, dashboards, and reporting tools.
• Proficient in managing Profit and Loss statements.
• Ability to adapt to continuous change and perform effectively under pressure in a dynamic and challenging environment.
• Strong problem-solving skills, critical thinking abilities, and effective collaboration skills.
