
EAP Care Advocate, Licensed Counselor/Social Worker

This is a fully remote position, open to applicants in United States.
• Offer guidance, support, consultation, and referrals to eligible employees and their dependents
• Perform comprehensive clinical assessments, which include identifying presenting issues, work-life challenges, and safety and risk concerns
• Create personalized care plans that address each participant’s clinical and sub-clinical needs
• Coordinate referrals and assist participants in securing appointments for short-term EAP counseling
• Provide mental health advocacy and navigation for participants requiring higher levels of care
• Aid in referrals to additional employer-sponsored health management resources
• Engage in active case management, including reviews of treatment plans
• Follow up with participants to ensure connection to care, satisfaction with the program, and assess EAP service outcomes and impact
• Keep accurate and complete records in accordance with HIPAA, ethical standards, and departmental guidelines
• Master’s degree in a mental health field, such as social work, counseling, or marriage and family therapy
• Relevant post-graduate clinical experience
• Clinical licensure
• Familiarity and comfort with technology for training and remote work, including Microsoft Office Suite, contact management systems, web-based software, and soft phone
• Strong analytical skills to collect data, establish facts, define problems, and draw valid conclusions
• Ability to excel in a fast-paced, fully remote work setting
• Previous experience in EAP and/or case management is a plus
