Counselor, Psych@Home

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Provide individual, group, or family therapy sessions for both adults and children.

• Carry out comprehensive diagnostic evaluations and continually assess suicide risk, implementing safety planning as necessary.

• Orient patients to the Psych@Home program and initiate discharge planning at the commencement of treatment.

• Collaborate regularly with internal and external provider partners as clinically required.

• Ensure client documentation is maintained in compliance with Medicaid, JCAHO, ODADAS, ODMH, CARF, and professional standards.

• Prepare all necessary documentation for discharge and aftercare referrals.

• Actively participate in and contribute to team rounds within the Psych@Home program.

• Take initiative to understand and protect the welfare of others.

• Engage proactively in supervision with your Supervisor; consult with supervisor regarding ethical and legal matters related to the work.

• May be required to provide in-person services or attend in-person meetings (at a clinical site) as needed, as determined by Signature Health.

• Responsible for fulfilling all productivity and documentation requirements.

• Participate in after-hours calls to accept potential admissions.

⛳️ Requirements

• Bachelor’s Degree is mandatory.

• A valid, unencumbered LSW, LISW, LPC, LPCC, LIMFT, or LMFT License in the state of Ohio is required.

• Certification in Basic Life Support (BLS) from the American Heart Association (AHA) is required.

• Proficiency in treating mental health and/or substance use disorders within individual, group, couples, and family therapy settings.

• Experience in collaborating with team members to enhance client outcomes.

• Knowledge of state and federal healthcare regulations.

• Awareness of community and state support resources relevant to the population served.
