Community Resource Coordinator II

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Facilitate community connection initiatives by linking members to community resources throughout their care management experience.

• Deliver cost-effective community and care resources to members.

• Organize community outreach efforts and increase awareness of available care and services.

• Assist members with inquiries regarding community and care resources.

• Support member communication and the distribution of educational materials.

• Record and manage community resources in compliance with health plan regulations.

• Aid nurses and social workers in outreach, community education, informal guidance, and member assistance.

• Perform non-clinical general health evaluations and refer members to suitable care or services.

• Carry out non-medical assessments related to home safety, community/environment resources, transportation, employment, and other essential needs.

• Engage in telephonic and/or in-person outreach to connect with hard-to-reach individuals and families.

• Visit individual homes and/or community organizations as required.

• Undertake other assigned responsibilities and adhere to policies and standards.

⛳️ Requirements

• High School diploma or GED.

• 1–2 years of relevant experience.

• Familiarity with Social Determinants of Health (SDOH) barriers.

• Capability to conduct non-clinical general health assessments.

• Proficiency in conducting non-medical assessments, including those related to home safety, transportation, employment, and community resources.

• Ability to perform telephonic and/or in-person outreach.

• Capacity to support pediatric behavioral health foster care members.
