This is a fully remote position, open to applicants in Missouri.
• Act as a community-oriented, patient-centric resource, fostering relationships with patients and caregivers through direct, in-person re-engagement within designated geographic regions.
• Offer practical assistance with program re-enrollment, onboarding processes, and mobile application setup and usage.
• Conduct telephonic outreach to caregivers to facilitate the re-engagement of children in care and assist with onboarding.
• Inform patients and caregivers about the services offered by Imagine Pediatrics and the available community resources.
• Stay informed about local community resources and connect families to suitable services.
• Organize community events and perform facility rounding.
• Collaborate with Behavioral Health teams and local behavioral health providers on discharge planning and community resource linkages.
• Work alongside caregivers, families, and facility staff to identify and mitigate barriers to engagement and access to care.
• Partner with the Patient Engagement Team, Outreach Specialists, and Registered Nurses to achieve re-engagement objectives.
• Assist with non-clinical care coordination, including primary care connections, pharmacy access, discharge and follow-up support, addressing SDOH needs, and eliminating barriers to care.
• Facilitate outbound engagement calls for enrollment, follow-ups, and patient engagement metrics.
• Possession of a valid driver’s license.
• Bachelor’s degree required.
• Willingness to travel frequently.
• Minimum of 3 years of experience in a healthcare environment, specifically in a patient-facing role within a hospital or facility.
• Capability to navigate uncertainty, think critically, take initiative, and maintain organization.
• Proficient in managing multiple technology platforms.
• Experience in a patient-facing role within a hospital or facility is required.
• Experience collaborating as part of an interdisciplinary team.
• National Community Health Worker Certification is highly preferred.
• Bilingual ability in Spanish is highly preferred.
• Medical Interpreter Certification is preferred.
