This is a fully remote position, open to applicants in Colorado.
• Deliver training, coaching, and technical support to community health centers to enhance outreach programs, quality improvement efforts, and workforce development.
• Evaluate outreach and engagement metrics, oversee program performance, and assist with grant compliance and reporting.
• Supervise, mentor, and nurture Community Outreach Specialists and related personnel, including recruitment, onboarding, performance management, and professional growth.
• Collaborate with both internal and external partners to establish workflows, policies, and best practices.
• Assist patients in preventive care and chronic condition management through education, care navigation, appointment scheduling, and resolving barriers.
• Recognize and tackle social and logistical obstacles to care, connect patients with community resources, and facilitate referral follow-up and documentation.
• Engage in interdisciplinary care teams, quality improvement initiatives, and program planning.
• Ensure adherence to organizational policies, HIPAA regulations, patient safety standards, and expectations of grant-funded programs.
• High school diploma or GED is mandatory.
• Associate degree or relevant certification is preferred.
• Three to five years of experience in community health, care coordination, outreach, population health, or public health programs is required.
• Experience in leading community health worker activities is essential.
• Prior supervisory or team leadership experience is necessary.
• Experience working with diverse, underserved, and community-based populations is required.
• Basic knowledge of medical terminology and healthcare systems is necessary.
• Proficiency in electronic health records, care management platforms, and mobile documentation tools is essential.
• Strong organizational, communication, and problem-solving abilities are required.
• Experience in writing, organizing, reporting, and presenting data from operational and/or research activities is crucial.
• Credential from the Colorado Department of Public Health & Environment Community Health Worker Registry or the ability to obtain it within a specified timeframe is preferred.
• Experience in performing or supporting basic clinical screening activities, such as blood pressure measurement or point-of-care testing, is preferred.
• Familiarity with community organizing, community engagement, or community-based participatory research is preferred.
• Knowledge of existing community resource networks is advantageous.
• Bilingual or multilingual proficiency reflective of the communities served is preferred.
