Clinician Field Mentor, Nurse Practitioner / Physician Assistant

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Conduct In-Home Evaluations, Focused Visits, and Video Visits.

• Evaluate new diagnostic and preventive services along with clinical workflow modifications, offering constructive feedback.

• Ensure 40 hours of availability weekly and maintain a capacity of over 128 units each month.

• Dedicate one day a week for coaching and mentorship initiatives.

• Achieve a minimum of 88 completed visits per month.

• Mentor new-hire clinicians throughout their initial 90 days.

• Organize virtual and in-person shadowing sessions.

• Share effective strategies on clinical workflows and diagnostic/preventive services.

• Provide collective support to active clinicians through webinars and office hours.

• Deliver real-time assistance to clinicians in the field.

• Address non-urgent clinical inquiries initially and escalate issues to Clinical Leaders or Operations Managers as necessary.

• Regularly meet with Clinical Leaders for guidance and training.

• Engage in managerial, team, and clinical meetings.

• Test and finalize diagnostic/preventive service products as instructed.

• Ensure adequate supplies and diagnostic equipment are maintained.

• Travel regionally and locally based on business requirements.

• Accommodate field observations and ride-along experiences.

⛳️ Requirements

• Clinical background and appropriate state licensure, such as MD/DO or NP/PA.

• Capability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds of supplies.

• Ability to inspire, motivate, and mentor colleagues.

• Skill in sharing best practices and practical advice.

• Understanding of managing inquiries and escalating performance issues to leadership.

• Knowledge of maintaining high-quality member care across teams.

• Experience in conducting In-Home Evaluations aligned with Signify Health operational, clinical, and member-experience KPIs.

• Experience in mentoring teams and providing individualized professional and personal support.

• Familiarity with Signify Health In-Home Evaluation tools and processes.

• Experience in developing and delivering group presentations.

• Preferred multi-state licensure.

• Preferred 5–7 years of clinical experience, particularly in primary care, chronic disease management, or related fields.

• Preferred at least 6 months of experience performing Signify Health in-home health evaluations.

• Maintain training, affiliation, and compliance with Video Visit standards.

• Complete all diagnostic and preventive service products as required.

• Provide rotating evening and weekend mentoring/coaching support as assigned.

• Flexibility to travel locally and regionally.

• Ability to facilitate field observations and ride-along experiences.
