This is a fully remote position, open to applicants in North Carolina.
• Deliver clinical staffing assistance across regions with high demand for Well Care Home Health services.
• Offer direct nursing care and case management support during periods of vacancy, fluctuations in patient census, and other staffing requirements.
• Evaluate patients as well as their families/caregivers, completing initial assessments, comprehensive evaluations, ongoing reassessments, and recertification tasks.
• Collaborate with physicians, patients, families/caregivers, and interdisciplinary teams to develop, implement, and revise personalized care plans.
• Oversee care coordination and supervise assigned aides, LPNs, and delegated services.
• Relay patient needs, changes in conditions, responses to treatment, orders, safety concerns, and care transitions to physicians and agency leadership.
• Maintain caseloads, visit responsibilities, documentation, clinical notes, and all necessary forms.
• Travel to assigned locations and counties to provide temporary coverage for PTO, call-outs, FMLA, and areas without coverage.
• Engage in interdisciplinary conferences, performance improvement initiatives, education, and care coordination.
• Foster patient satisfaction, uphold agency standards, maintain confidentiality, adhere to ethical practices, ensure safety, and comply with regulatory standards.
• Graduate from an accredited or approved nursing program with an AD, Diploma, or BSN degree.
• Hold a current license to practice professional nursing in the state where care is delivered.
• Multi-state licensure or the capability to obtain licensure for assigned service areas may be necessary.
• Current CPR certification is required.
• A minimum of two years of RN experience is necessary.
• Home Health experience is mandatory.
• Ability to work independently and adapt to diverse clinical settings.
• Capability to manage patient care responsibilities and coordinate interdisciplinary care effectively.
• Possess hand/eye coordination for injections, proficiency with computer and electronic medical record systems, patient care equipment, and clinical procedures.
• Must be able to communicate and be literate in English.
• Ability to handle patient care equipment and assist in transferring and guarding patients.
• Capability to lift or maneuver objects weighing at least 50 pounds.
• Ability to assess patient conditions, develop care plans, choose interventions, evaluate responses, and educate patients.
• Ability to visually assess patients, auscultate sounds, hear alarms, and communicate effectively.
• Flexible hours that may include Monday to Friday, weekends, holidays, and on-call participation as needed.
• Valid driver's license, access to an operational vehicle, and the ability to travel across assigned Home Health locations.
• Capability to serve both adult and geriatric populations.
• Willingness to work in environments that involve bloodborne pathogens, bodily fluids, infectious diseases, needle puncture risks, animals, traffic, weather extremes, and potential safety hazards.
