This is a fully remote position, open to applicants in United Kingdom.
• Conduct comprehensive medicines reconciliations utilizing clinical documentation, including discharge summaries from hospitals and outpatient letters.
• Detect and address inconsistencies between existing medication records and the latest hospital communications.
• Evaluate and record changes in medications, initiating necessary actions or flagging for follow-up with the GP when needed.
• Collaborate remotely with general practitioners, care coordinators, and practice teams to ensure safe and precise prescribing.
• Utilize clinical systems and decision-support tools to assess for possible drug interactions, duplications, or contraindications.
• Ensure accurate clinical documentation that aligns with practice protocols and professional standards.
• Participate in continuous professional development to maintain current knowledge of medicines optimization and best practices in primary care.
• Must be a registered pharmacist with registration from the GPhC.
• Must be an independent prescriber.
• Must have successfully completed the CPPE pathway.
• Exceptional analytical and problem-solving skills with a keen attention to detail.
• Strong verbal and written communication skills for effective interactions with patients and team members.
• Proven ability to work independently and handle multiple tasks remotely.
• Starting from £25 per hour
