This is a fully remote position, open to applicants in Arizona, +10 more states.
• Oversee a diverse team of professionals, including Registered Nurses, Licensed Vocational Nurses, Social Workers, Chaplains, and Medical Assistants.
• Engage in strategic planning to influence the future direction of clinical care delivery.
• Lead and participate in significant initiatives aimed at enhancing clinical care, quality, and introducing new programs.
• Identify, troubleshoot, and address clinical workflow challenges that adversely affect care quality or operational outcomes.
• Ensure sufficient and effective patient care coverage.
• Assist in achieving key performance indicators (KPIs) assigned to the care delivery team and conduct annual performance evaluations.
• Collaborate with Providers and multidisciplinary professional staff to improve patient care, implement changes, and foster process enhancements.
• Provide leadership in conflict resolution, mentoring, skilled decision-making, time management, adaptability, and commitment.
• Deliver direct patient care as necessary to align with organizational objectives.
• Aid in the development and onboarding of newly hired clinical staff (RN, LVN, SW, Provider, MA, etc.).
• Ensure the team's compliance with regulations (state license renewals, certification check-offs, etc.).
• Up to 25% travel may be required for training, meetings, and other business-related activities.
• Possession of an active, unrestricted LCSW license in California, with the capability to obtain additional state licensure as required.
• Minimum of 2 years of experience in people management.
• At least 5 years in a direct care delivery position as a Licensed Clinical Social Worker.
• A minimum of 2 years of experience in palliative care, hospice care, or complex care management.
• Strong understanding of cultural competency related to the target population.
• Demonstrated, results-oriented leader with experience in change management within rapidly growing environments.
• Excellent organizational skills and the ability to work independently.
• Previous remote work experience with proficiency in Microsoft and Google applications; familiarity with Salesforce is an advantage.
