Clinical Case Manager

This is a fully remote position, open to applicants in Arizona, +9 more states.

📋 Description

• Evaluate member requirements in various domains including physical health, mental health, substance use disorder (SUD), oral health, palliative care, memory care, trauma-informed care, social supports, housing, and connection to community-based services.

• Supervise the creation of client care plans and the establishment of goals.

• Provide services at locations convenient for the member, whether it be in-person, via telehealth, or in the field.

• Link clients to essential social services and supports.

• Advocate for the client with healthcare professionals (e.g., primary care providers).

• Apply evidence-based methodologies, including Motivational Interviewing, Harm Reduction, and principles of Trauma-Informed Care.

• Perform outreach and engagement initiatives to promote connection to the ECM program and document activities in the Client Relationship Management (CRM) system.

• Assess client progress and revise SMART goals accordingly.

• Facilitate mental health promotion.

• Coordinate transportation services (e.g., ACCESS).

• Complete all necessary documentation, including outcome measures, within the timelines specified in individual care plans.

• Keep patient health records current in the Electronic Medical Record (EMR) system and other relevant business systems.

• Generate monthly reports to ensure adherence to program compliance.

• Participate in assigned training sessions.

⛳️ Requirements

• Must possess an active, unrestricted LCSW, LMFT, or LPCC license in California.

• At least 1 year of experience as a care manager, care navigator, or community health worker for vulnerable populations.

• Familiarity with government and community resources related to social determinants of health.

• Strong oral and written communication abilities.

• Positive interpersonal skills are essential.

• General computer proficiency and knowledge of Google Workspace, MS Office, and the internet are required.

• Bilingual proficiency in English and Spanish is highly preferred.
