Clinical Care Coordinator, LPN/BSW

This is a fully remote position, open to applicants in Washington.

📋 Description

• Assists in coordinating care management activities under the supervision of an RN or LICSW Case Manager, Supervisor, or Manager.

• Operates as an interdependent team member focusing on member assessment, planning, facilitation, and advocacy.

• Communicates with healthcare providers to schedule follow-up appointments, acquire clinical records, and aid members in care coordination.

• Ensures the effective coordination of services to members, including addressing transportation needs and connecting them with community resources like food pantries and housing.

• Coordinates and facilitates services with both physical and behavioral healthcare providers during an episode of care.

• Documents and oversees the care management process, which encompasses case identification, assessment, planning and goal prioritization, monitoring, support, and case closure.

• Engages in the monitoring and collection of clinical outcome data.

• Evaluates member health care status and health history (medical, behavioral, and social), including risks, gaps, and utilization.

• Develops personalized care plans with appropriate member-centric interventions.

• Provides behavioral health support and interventions for members in crisis.

• Collaborates with the member's PCP, specialists, and community resources/vendors to maximize available community resources.

• Ensures that all care coordination activities are documented in the medical record system according to established guidelines.

• Supports staff in managing member calls, including participating in the 5 PM call rotation for crisis situations.

⛳️ Requirements

• Must hold a Bachelor’s degree in social work, a Practical Nursing Certificate, or a comparable bachelor’s degree in a relevant healthcare field.

• Candidates with a bachelor’s degree in a relevant healthcare field should have a minimum of one (1) year of experience in care coordination or care management.

• Previous experience as a medical assistant, behavioral health technician, or in other social welfare services is required.

• At least one (1) year of clinical and/or care management experience is mandatory.

• Must possess proficiency and experience with Microsoft Office applications.

• Knowledge of Medicare and Medicaid regulations is essential.

• One (1) year of experience in care management within a health plan, community agency, or Accountable Care Organization (ACO) is preferred.

• Familiarity with care management systems and planning techniques is required.

• Experience with care management workflow systems is preferred.
