This is a fully remote position, open to applicants in Texas.
• Assign primary diagnosis and procedural codes for designated outpatient and ambulatory care settings.
• Code diagnoses, procedures, and services for emergency departments, outpatient clinics, same-day surgeries, diagnostic testing, radiology, and laboratory services.
• Review medical record documentation to identify relevant diagnoses and procedures that require code assignment.
• Accurately assign ICD-10-CM, modifiers, and HCPCS codes for purposes of reimbursement, research, and regulatory compliance.
• Review records for the specificity of diagnoses and procedures as well as appropriate reimbursement for hospital and professional charges.
• Query physicians when documentation is inadequate, ambiguous, unclear, or when coding assignments are not straightforward.
• Stay updated with coding and reimbursement reporting guidelines and escalate any concerns to the manager.
• Maintain a coding accuracy rate of at least 95% for ICD-10-CM and/or HCPCS while meeting productivity standards.
• Ensure adherence to AHIMA ethical coding standards and official coding guidelines.
• Follow Memorial Hermann's policies, procedures, safety standards, budget specifications, and quality of service expectations.
• Complete mandatory and continuing education and uphold skills competency.
• Support departmental goals and act as a preceptor, mentor, and resource for less experienced staff.
• Provide safe, caring, personalized, and efficient experiences for patients and colleagues.
• Perform additional duties as assigned.
• High School Diploma or GED is required.
• RHIT, RHIA, or relevant coding certification(s) from AHIMA or AAPC are required.
• A minimum of three years of hospital or ambulatory coding experience is required for Surgery/Observation.
• At least two years of hospital or ambulatory coding experience is required for Emergency Department.
• Two years of hospital or ambulatory coding experience is required for Interventional Radiology.
• One year of hospital or ambulatory coding experience is preferred for Ancillary/Recurring.
• Experience coding in a level 1 trauma facility or an academic teaching facility is preferred.
• Effective oral and written communication skills are necessary.
• Strong knowledge of ICD-10-CM and/or CPT is essential.
• Analytical skills for interpreting health record data and assigning appropriate codes are required.
• Proficient knowledge of human anatomy, physiology, medical terminology, and surgical terminology is necessary.
• Critical thinking, sound judgment, and decision-making skills are essential.
• Knowledge of coding compliance policies, official coding guidelines, regulatory requirements, and internal policies and procedures is required.
• Proficiency in navigating a Windows-based application environment is necessary.
• Must maintain a coding accuracy rate of 95% or higher for ICD-10-CM and/or HCPCS.
• Must consistently meet established productivity standards.
• Must adhere to AHIMA Standards of Ethical Coding and official coding guidelines.
