This is a fully remote position, open to applicants in New Jersey.
• Create proactive care plans aimed at addressing identified challenges and enhancing both short- and long-term outcomes.
• Assess, strategize, implement, and manage case management activities.
• Evaluate the medical needs and overall wellness of members.
• Review clinical tools, data, and claims to determine member needs and benefits.
• Utilize clinical judgment to mitigate risk factors and address complex health and social indicators.
• Conduct comprehensive assessments that include comorbidities, multiple diagnoses, and functionality.
• Identify the necessity for referrals to clinical resources and members of the interdisciplinary team.
• Collaborate with supervisors, healthcare stakeholders, providers, community organizations, and interdisciplinary teams.
• Present cases during interdisciplinary case conferences.
• Employ case management processes in alignment with regulatory and company policies.
• Utilize motivational interviewing techniques to engage members and evaluate their health status and needs.
• Meet with members in person within the designated Bergen/Hudson County area.
• A minimum of 3+ years of clinical practice experience is required.
• Active and unrestricted RN licensure in New Jersey is mandatory.
• Willingness and ability to travel 25–50% using personal vehicle for face-to-face member visits.
• Reliable transportation is necessary.
• Must live near or within Bergen/Hudson County, New Jersey.
• A dedicated workspace free from interruptions is essential.
• Separate dependent care arrangements during work hours are required.
• Certification as a Case Manager is preferred.
• A minimum of 2+ years of experience in care management, discharge planning, and/or home health care coordination is preferred.
• Ability to work independently and think critically from home.
• Strong analytical and problem-solving capabilities are essential.
• Effective communication, organizational, and interpersonal skills are required.
• Proficient computer skills, including the ability to navigate multiple systems and keyboarding, are necessary.
• Proficiency in MS Word, Excel, Outlook, PowerPoint, and proprietary applications is required.
• Bilingual ability is preferred.
• An Associate’s degree is required.
• A Bachelor’s degree is preferred.
