Care Partner, Certified Medical Assistant

This is a fully remote position, open to applicants in United States.

📋 Description

• Develop and monitor care plans for patients.

• Support patients throughout their healthcare experience.

• Frequently engage with patients to track their compliance with care plans.

• Diligently observe patients' device readings and investigate any abnormal values.

• Keep detailed records of patient interactions and program information.

• Remain informed about the latest regulations and compliance requirements related to CCM/RPM services.

• Ensure all patient enrollments meet legal and ethical guidelines.

• Evaluate and track care performance, aiming to achieve or surpass set goals.

⛳️ Requirements

• National Certification in Medical Assisting.

• Must be a resident of the US.

• Excellent communication abilities, both written and spoken.

• Compassion and the capacity to connect with patients and their families.

• Fundamental understanding of chronic health issues and their management.

• Proficient in using technology for documentation and communication purposes.

• Strong attention to detail and exceptional organizational skills.

• Capability to work autonomously as well as collaboratively within a team.
