This is a fully remote position, open to applicants in Illinois.
• Oversee physiological indicators such as blood pressure, weight, and blood glucose with clinical accuracy.
• Achieve team objectives and standards as defined by metrics.
• Make a significant contribution to sustained patient engagement within RPM program(s).
• Offer preventive health education and coaching for disease management.
• Conduct monthly wellness calls with assigned patients.
• Facilitate collaborative wellness calls with patients to establish health goals set by their Care Team.
• Manage patient communications and alerts.
• Guide patients to their treating physician for routine inquiries.
• Attain goals related to patient engagement programs.
• Adhere to appropriate escalation procedures for any urgent care requirements.
• Unrestricted RN/LPN/LVN license in a compact state, with a preference for Illinois.
• Ability to work in the CST zone from 8:00 AM to 5:00 PM.
• Spanish language proficiency is a plus.
• 1-3 years of clinical experience, patient management, or disease management is preferred.
• Familiarity with various provider practices and workflows.
• Quick learners are encouraged to apply.
• Capability to work independently with little supervision.
• Experience with Medicare patients is preferred.
• Background in conducting virtual patient visits and telephonic care management.
• Interest in professional leadership growth and development opportunities within a dynamic organization.
• Enthusiasm for participating in a new, innovative clinical program and being a key contributor to the establishment of processes and best practices for RPM patient care.
• Experience in Motivational Interviewing/Health Behavior Change is a plus.
• Health Coach certification is an advantage.
