Care Navigator

This is a fully remote position, open to applicants in New York.

📋 Description

• Conduct virtual consultations with patients, emphasizing education on lifestyle and behavior related to their clinical conditions.

• Manage a designated patient load based on the needs of the practice.

• Review and respond to remotely transmitted patient data, such as blood pressure, blood glucose, weight, and pulse oximetry, in accordance with approved partner protocols.

• Document all patient communications in their respective records.

• Assist patients in setting up their devices.

• Maintain communication with enrolled patients alongside their care teams.

• Build relationships with assigned patients to promote self-management of chronic illnesses and enhance overall health.

• Collaborate with team members when additional support is required.

• Carry out other responsibilities as assigned.

⛳️ Requirements

• A current, valid, and in-good-standing New York Nursing License (LPN/LVN) is required.

• A current, valid, and in-good-standing Multistate/Compact Nursing License is preferred.

• At least 3 years of experience in cardiology practice is preferred, or experience in primary care, home care, or chronic care management.

• Preferred experience in remote monitoring and/or virtual care management programs.

• High level of computer literacy and comfort in learning and utilizing a digital health portal.

• Ability to work independently.

• Strong time management and organizational skills.

• An engaging personality and a willingness to learn new topics in lifestyle medicine.

• Capability to acquire, interpret, and communicate clinical research effectively.

• Meticulous attention to detail.

• A HIPAA-compliant remote work environment is required.

• A distraction-free workspace is essential.

• Reliable internet connection with a minimum of 100 Mbps download and 10 Mbps upload speed is required.

• New York residency or license eligibility is indicated by the role's specific New York requirement.
