Care Manager – Registered Nurse

This is a fully remote position, open to applicants in Missouri.

📋 Description

• The Care Manager—Registered Nurse is responsible for coordinating care for members with chronic health conditions.

• Perform thorough assessments and address the social determinants affecting health.

• Create and execute personalized care plans.

• Track member progress and advocate for necessary services.

• Work collaboratively with an interdisciplinary care team.

• Offer education and support for disease management.

• Ensure precise documentation of all care management activities.

⛳️ Requirements

• Must possess an active and unrestricted Registered Nurse (RN) licensure in the state of MO or compact licensure in the state of residence.

• Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams.

• Have access to a private, dedicated space for effective work performance.

• Minimum of 3+ years of nursing experience required.

• At least 2+ years of experience in case management, discharge planning, or home healthcare coordination is necessary.
