Care Manager – Behavioral Health

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Develops, evaluates, and facilitates intricate care management activities focused primarily on mental and behavioral health requirements.

• Assesses the needs of the member through phone consultations or in-home visits.

• May conduct telephonic outreach to evaluate member requirements.

• Formulates ongoing care plans for members exhibiting high levels of acuity.

• Coordinates effectively between the member and/or their family/caregivers, community resources, and the care provider team as needed.

• Monitors care plans, service plans, and/or the status and outcomes of members.

• Facilitates care coordination and collaborates with relevant providers or specialists.

• Collects, documents, and maintains comprehensive member information and care management activities.

• Educates members and their families regarding procedures, healthcare provider instructions, treatment options, referrals, and healthcare benefits.

• Performs additional duties as assigned.

⛳️ Requirements

• A Master's degree in Behavioral Health or Social Work, or a degree from an accredited nursing school is required.

• 2 to 4 years of relevant experience is necessary.

• A licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN, in accordance with state contract requirements, with experience in behavioral health is required.
