Care Manager

This is a fully remote position, open to applicants in Maryland.

📋 Description

• Provide oversight for care management programs.

• Responsible for recruiting and selecting suitable staff while preparing for timely employee performance evaluations.

• Foster individual and team growth, offering performance management as needed.

• Prepare reports outlining key indicators and activities for committees and senior management.

• Collaborate with the Director of CM to develop the annual budget and ensure operation within the approved budget.

• Work with interdisciplinary teams to assist staff in coordinating care activities.

• Act as a resource for health plan information and facilitate the integration of care management functions throughout the health plan.

• Aid in the annual establishment of measurable goals for care management programs based on data from outcome reports, quality indicators, regulatory agencies, and health plan initiatives and priorities.

• Implement continuous development and education for care management staff.

• Ensure program integrity and compliance with regulatory requirements in collaboration with the Director of CM.

⛳️ Requirements

• Current state RN license is required.

• Bachelor of Science in Nursing (BSN) is preferred.

• At least two years of management or supervisory experience is required.

• A minimum of three years of care management experience is required.

• CCM Certification is preferred.

• Experience in Managed Care is preferred.
